Place your completed and signed DOJ Amendment form and supporting documentation (e.g. qualification certificate, utility bill, and/or legal name change document) in an envelope, including a check in the amount of $10.22 payable to OCSD (Cash, credit/debit cards are not accepted). Mail the sealed envelope to the following address:

Upon receipt, the information will be verified and a new license will be sent via US Mail to the address listed on your CCW License along with a return envelope for your originally issued license.
Note: Failure to return the originally issued license is a violation of the Terms of License Agreement and may result in the revocation.
Contact the CCW Licensing Unit via the online CCW Appointment Request feature at ocsd.org/ccw to request an appointment. On the day of your appointment, bring your signed DOJ Amendment form and supporting documentation (e.g. qualification certificate, utility bill, and/or legal name change document) in an envelope, including a check in the amount of $10.22 payable to OCSD (Cash, credit/debit cards are not accepted). During your appointment, the amendment will be processed and a new license will be issued. Please remember to bring your current license with you which will be exchanged for your amended license.
